Because of limited resources, the Wellness Qlinic is not able to offer urgent care. If you have any urgent care needs, please visit your local urgent care center or call 1-888-NYC-WELL.
If you are in an emergency, please call 911 or 1-888-NYC-WELL or visit your local emergency room.
Information About Your Recent Visit
If you have any questions or concerns about a recent appointment, please contact us at (646) 962-9297 and leave a message with your name, date of birth, phone number, and the reason for your call. A member of the care team will return your call within 72 hours.
Scheduling an Appointment
If you are a current patient and want to schedule a follow-up appointment, please call us at (646) 962-9297 and leave a message with your name, date of birth, phone number, and tell us that you are calling for a follow-up. A patient coordinator will call you within one week.
Rescheduling an Appointment and Cancellations
If you need to reschedule an appointment, call the Wellness Qlinic at (646) 962-9297 and leave a message with your name, date of birth, phone number, the date of the appointment, and when you would like to reschedule. A patient coordinator will follow-up with you within 72 hours.
Please note our Appointment Policy when rescheduling appointments. For initial evaluations and first-time visits, patients are permitted to reschedule their appointment once.
Due to demand for and limited supply of appointments, current patients must attend all scheduled appointments or risk losing access to services. Patients are allowed three missed appointments and/or late cancellations before services may be restricted or denied.
Appointments canceled or rescheduled later than 48 hours (2 days) in advance of the appointment will be noted and counted late in accordance with the Attendance Policy.
Accessing Your Medical Records
Patients may request copies of their medical records by completing the Medical Record Request Form. Download the form or pick up a copy from the Wellness Qlinic during clinic hours.
Once completed, you must fax us the form at (646) 962-0097. Records can be picked up during Qlinic hours or sent to you via fax or mail.
What to Do if You Get a Bill
If you get a bill from NewYork-Presbyterian Hospital (NYPH) or Weill Cornell Medical Center (WCMC) for Wellness Qlinic services, please let us know as soon as possible.
All services provided by the Wellness Qlinic are offered free of charge, but rarely a bill may be generated. We aim to resolve these bills as quickly as possible; however, we require your assistance in notifying us of a bill as soon as possible, as we may not otherwise be aware.
Note that the Wellness Qlinic waives costs for all services offered at the Wellness Qlinic; we do not cover the cost of any referrals, emergency room visits, pharmaceutical costs, or other outside care.
If you receive a bill that you believe is in error:
- Call us at (646) 962-9297 to report the error. Please leave a message with your name, date of birth, account number, date of service, and amount billed.
- Fax us the bill at (646) 962-0097.
- If you still receive bills after 2 months, please call us again to let us know.
If you receive a call from a collection agency regarding care at NYPH or WCMC:
- Identify the date of service for the bill.
- If the date corresponds to a Wellness Qlinic appointment, inform them that services offered by the Wellness Qlinic should not be billed and direct them to call the Wellness Qlinic at (646) 962-9297.
- Call us at (646) 962-9297 to report the collection call. Please leave a message with your name, date of birth, account number, date of service, and amount billed.
