To request an appointment as a new patient at the Weill Cornell Medicine Wellness Qlinic, please fill out our online form.
Because of our limited resources, an appointment request here does not guarantee an in-person evaluation and an evaluation does not guarantee ongoing treatment.
After submitting the request form, a patient coordinator will contact you within one week to follow up. If you have any special instructions for calling, such as a preferred time or concerns about someone else answering your phone, please include those in your description of the reason for your appointment on the form.
